Vaccine Clinic Survey | Submit by Feb. 2
Posted: January 29, 2021
Dear Gannon Community,
As a next step in our fight against COVID-19, Gannon University and LECOM have partnered to provide a COVID-19 Vaccine Clinic that will be offered to the Gannon community beginning in February. Details of this clinic will be announced in the days ahead as planning progresses.
To aid in the planning and roll-out of the clinic, we are asking that all students, faculty and staff please complete the brief survey at the link below by noon on Tuesday, Feb. 2, to help us determine our needs and how best to serve the Gannon community.
Gannon/LECOM COVID-19 Vaccine Clinic Survey
We are working diligently with our colleagues at LECOM to bring the COVID-19 Vaccine Clinic to campus. We will adhere to the Pennsylvania Department of Health guidelines to prioritize providing this opportunity to our most at-risk populations in our first sessions, then opening it to the broader Gannon community in the sessions to follow.
Completing this survey will identify those with high-risk factors who would like to receive the vaccine so that we may provide an invitation and instructions for priority scheduling.
Whether you would like to receive the vaccine, do not wish to receive it, or have already received part of or both doses of the vaccine, please complete the survey to share that information with us. This brief survey is less than 10 questions and will take just a few minutes to complete.
Please submit your survey by noon on Tuesday, Feb. 2 to provide our team the time to process survey submissions and roll-out this clinic to our community in the most efficient manner possible.
Thank you in advance for your participation in completing this survey. We appreciate your active role in continuing to keep our community a safe and healthy place to learn, live and work.
